There are two ways to apply for massage chair financing for one full year: a quick and easy online application, or a mail/fax application. For the easiest and fastest way to qualify for your massage chair, we encourage you to apply online! For applying by mail or fax, please follow the instructions under "Paper financing procedure". As soon as you are approved, we will contact you to arrange a delivery time for your new massage chair!

Quick online financing procedure:

Complete the online financing form. Your data is completely safe and encrypted using 128-bit SSL for maximum security. You will need a driver's license and credit card to verify your identity.

Paper financing procedure:

1) Download, print and fill out the PDF application form, or fill it out on screen and then print. Click here for the application.  (Adobe Acrobat is required)
2) Sign and fax back to 303-395-1326 with a copy of your driver's license and a copy of any valid credit card, these are used only to verify you are who you say you are, and will help us to prevent identity theft. We cannot run your application without proof of identity.
3) Please read "Part 2": Credit terms of the HOME DESIGN agreement before you sign the application.
4) You will then be notified of your approval and when you would like your chair delivered.
